My office is located at 63 Park Avenue, Rochester, NY 14607, and there are designated parking spots behind the building. 
 
Prior to your first appointment, I will contact you via email with my paperwork. If you’re unable to complete the paperwork ahead of time you can complete it when you arrive (it will be on a clipboard in the waiting area), but please allow for about 10 minutes prior to your appointment time.
